Scheduling

This service is offered to Gallaudet students and employees free of charge.

To schedule a Diagnostic Assessment, please complete the Assessment Request Form keeping the time blocks above in mind. Please print and read through the Informed Consent / Videorecord Release Form. You will need to bring a signed consent form to your first session. 

We do not recommend scheduling more than one Assessment within a six-month time period. If you are not currently a Gallaudet student or employee, Diagnostic Assessment is unfortunately not available to you at this time. We hope to offer this invaluable experience to individuals external to Gallaudet in the near future.

Cancellations/Missed Appointments Policy:

Except in cases of extenuating circumstances, individuals missing appointments without providing notice or canceling appointments less than 48 hours in advance may be assessed a fee.
